How would you manage this lesion on the helical rim?

In this week's case discussion, submitted by Dr David Stewart, we look at a 73-year-old male patient who presents for a routine skin check. This lesion is noted on his right helical rim. The patient is unaware of it.

Update

An excision biopsy was performed. The pathology result (below) reported basal cell carcinoma completely excised but with narrow margins. 

The patient was referred to plastics, and the wider excision came back as "keloid scarring with no residual tumour".
